Achieve the Intermediate Care Facility for Individuals with Intellectual or Developmental Disabilities (ICF/IDD) level of care, evaluated via the LT-104 assessment. b. Attain the nursing facility level of care, assessed through the LT-101 assessment.
  4. Meet the financial eligibility requirements assessed by the Medicaid Long Term Care Unit.
  5. Exhibit clinical eligibility through one of the following diagnoses, validated by an up-to-date psychological or neuropsychological evaluation: a. Intellectual disability. b. Developmental disability or a related condition. c. Acquired Brain Injury (ABI) following Chapter 1 of the Department of Health's Medicaid Rules.
  6. Fall within the age bracket of 21 to 64.
