  1. Community Reintegration: The MFP aims to support eligible individuals in reintegrating into their communities, allowing them to lead more independent and fulfilling lives.   2. MFP Community Living (MFP-CL) Waiver: The MFP-CL waiver caters to individuals who can move to their own home, apartment, or the home of someone else and receive the required services within the community. Eligibility Requirements

To be eligible for the Moving Forward Plan (MFP) Waivers in Massachusetts, individuals must meet specific criteria by MassHealth. The program aims to support those currently residing in nursing facilities or chronic disease, rehabilitation, or psychiatric hospitals and help them transition back to their communities with necessary resources and ongoing services.

To qualify for one of the MFP waivers, an individual must fulfill the following requirements:

  • Length of Stay: The applicant must have been living in a nursing facility or one of the specified institutional settings (chronic disease, rehabilitation, or psychiatric hospital) for a minimum of 90 days.
  • Age and Disability Status: The individual should be either 18 or older with a disability or 65 or older.
  • Clinical Requirements: The applicant must meet the clinical requirements to reside in one of the specified facilities.
  • Need for MFP Services: The individual should require the specific services offered by the MFP waivers to transition successfully into community living.
  • Safety in the Community: The individual must be deemed safe to be served in the community according to the terms and conditions outlined by the MFP waivers.
  • Financial Eligibility: The applicant should meet the financial requirements to qualify for MassHealth Standard in the community. Note that special financial rules exist for waiver applicants and participants.

For individuals seeking the MFP Residential Supports (MFP-RS) waiver, an additional criterion applies:

  • Residential Support Needs: The person must require residential support services with staff supervision 24 hours a day, seven days a week.

Compensation for Family Caregivers

Under the Moving Forward Plan (MFP) Waivers, compensation for unpaid family caregivers is possible through the self-directed option. The self-directed option allows eligible individuals to choose their caregivers, including family members, and pay them for the caregiving services provided. This empowers families to actively participate in their loved one's care while receiving financial support for their efforts.

By compensating unpaid family caregivers through the self-directed option, the MFP program recognizes these caregivers' invaluable role in supporting the transition to community living.
