The Community Choices Waiver (CC) is a Medicaid program that provides home and community-based services to elderly and disabled adults who require the level of care provided in a nursing facility. The CC waiver aims to help individuals remain in their homes or other community settings instead of institutional care. The CC waiver also offers a self-direction option, which allows participants to hire, train, supervise, and manage their personal care attendants, who can be family members. The CC waiver is administered by the Louisiana Department of Health (LDH) and the Louisiana Office of Aging and Adult Services (OAAS).

Eligibility requirements

To be eligible for the CC waiver, an individual must meet the following criteria:

  • Be a Louisiana resident
  • Be at least 21 years old
  • Be eligible for Medicaid
  • Have an income below 300% of the federal benefit rate (FBR), which is $2,382 per month in 2023
  • Resources below $2,000 for individuals or $3,000 for couple
  • Require the level of care provided in a nursing facility, as determined by a medical assessment
  • Choose to receive home and community-based services instead of institutional care
  • Have an approved plan of care that meets their needs and preferences

Services and supports

The CC waiver provides various services and supports to help individuals live independently and safely in their homes or other community settings. Some of the services and supports available under the CC waiver are:

  • Personal care services: Assistance with activities of daily living (ADLs) such as bathing, dressing, grooming, eating, transferring, and toileting
  • Respite care: Temporary relief for caregivers who need a break from their caregiving duties
  • Adult day health care: Supervised care in a group setting that provides social, recreational, and therapeutic activities
  • Home modifications: Changes to the home environment that improve accessibility, safety, and comfort
  • Assistive devices: Equipment or devices that enhance mobility, communication, or independence
  • Personal emergency response system: A device that allows individuals to call for help in case of an emergency
  • Transition services: Assistance with moving from an institutional setting to a community setting
  • Support coordination: Assistance with accessing and coordinating waiver and other community services

Compensation for caregivers

One of the benefits of the CC waiver is that it allows participants to self-direct their personal care services. This means they can hire, train, supervise, and manage their personal care attendants, who can be family members. Family members who provide personal care services under the CC waiver can get paid for caregiving. The payment rate for personal care attendants varies depending on the region and the level of service. According to LDH, the average payment rate for personal care attendants in 2021 was $9.50 per hour. However, this rate may change in 2023 due to inflation or budget adjustments.
