Arizona Health Care Cost Containment System eligiblity rules

To be eligible for Arizona Medicaid, you must be a resident of Arizona, a United States citizen or a qualified immigrant (if the person is not a U.S. citizen or qualified immigrant, they may still be eligible for emergency services), have a Social Security number or apply for one unless they cannot get one legally, apply for potential income that may be available, such as unemployment, pensions, and Social Security, and have family income under the income limits. You may also be one of the following:

  • Pregnant
  • Be responsible for a child 17 years of age or younger
  • Are Blind or have a disability or a family member in your household who is blind or with a disability

How to apply for Arizona Health Care Cost Containment System

You can apply online for AHCCCS Medical Assistance, Nutrition Assistance, and Cash Assistance using Health-e-Arizona Plus (HEAplus).

Alternatively you can download and submit a paper application. You can use one application to apply for AHCCCS Medical Assistance for everyone in your household. Download the application form in English or Spanish.

How to renew Arizona Health Care Cost Containment System

Medicaid recipients in Arizona will receive a renewal notice from the Arizona Health Care Cost Containment System (AHCCCS) approximately 90 days before their coverage expires, which includes instructions for the renewal process and the deadline for submitting the renewal application.

The renewal application can be completed online through Health-e-Arizona Plus (HEAplus), by mail, or in person at a local AHCCCS office. 

The application must be submitted before the deadline mentioned in the renewal notice. A new Medicaid ID card will be issued if the application is approved. If it's denied, there will be information on how to appeal the decision.
